Why is my child struggling to read? Signs of a hidden vision problem

The three key areas of eye function and how they affect reading skills

Has your child's school told you they're slow at reading, struggle with spelling, or can't copy from a book? Do they mix up their maths symbols or get tired quickly in class? If so, this blog will be of interest.  These struggles often have overlooked causes and understanding them can make a real difference. 

Three things affect how well a child reads and writes: 

  1. How well the eyes physically move - ocular motor skills and binocular vision.

  2. How the brain processes what the eyes see - visual processing.

  3. How that information is then understood - visual perception.

Vision is by far our most dominant sense. But seeing clearly doesn't automatically mean a child has the skills needed to read fluently, copy from a book or board, or check their own work. 

If an optician has ruled out the need for glasses, but difficulties still remain, it's worth looking at the three key areas of eye function. 

1. Ocular motor skills and binocular vision:

How the eyes physically move.

A set of muscles controls each eyeball. The strength and agility of these muscles affect basic eye movement (ocular motor skills) and how well both eyes work as a pair (binocular vision). 

Binocular vision is the ability of both eyes to work as a team, merging their two images into one. When this is unstable, double vision can result; sometimes one eye stops sending information to the brain to cope. Children with this issue often close one eye for certain tasks. 

Ocular motor skills cover how the eyes coordinate during different activities, and fall into three types: 

  • Pursuit, the smooth tracking movement the eyes make to follow something moving, like a ball, a bird, or a passing car. 

  • Saccades, small, quick jumps of the eyes as they fix on one point, then jump to the next.  

  • For a child learning to read, this is the jump from letter to letter as each sound is spoken aloud before moving on. If their eyes land on the wrong letter, the child can't decode the word properly; instead, they guess or rely on picture clues.  

  • For a more experienced reader, the eyes jump from word to word, or group to group. If this movement isn't smooth, the reader can lose their place, re-read the same words, skip ahead, or miss whole lines. 

  • Convergence and divergence, how the eyes adjust as something moves closer or further away, such as catching or throwing a ball. The eyes turn inward (converge) as an object approaches, and outward (diverge) as it moves away. 

Convergence is also essential for reading, since the eyes are constantly focusing on something close up. When convergence is insufficient, it can cause tired eyes, blurred vision, headaches, and even double vision if one eye moves at a different pace than the other while reading a page. 

Eye movement also works alongside the vestibular-ocular reflex, which keeps our gaze steady while the body is moving. It's the reason your view doesn't blur when you walk, or why you can fix your eyes on the horizon to stay steady on a rocking boat. 

When a child struggles with balance, it can knock their confidence in different forms of movement . Sometimes this stems from a weak ocular motor system; other times, poor coordination and gait keep that system from developing properly in the first place. 

This physical layer of eye movement is foundational. If you don't address it, everything that depends on it, processing and perception, will be affected too.

2. Visual Processing:

How the brain interprets what the eyes see.

Visual processing is the brain's job of turning a raw signal into something meaningful; sensory information becomes neural information. Once an image hits the retina at the back of the eye, it sends the information via the optic nerve to the brain's visual cortex. There, the brain flips the image the right way up and starts making sense of it. This happens in two stages: 

  • Low-level processing: where the brain distinguishes contrast, shape, orientation and colour. 

  • High-level processing: where the brain adds meaning, context, and understanding to what's being seen. 

When ocular motor skills are weak, this processing slows right down, because the brain has messier raw material to work with.

3. Visual Perception:

How children make sense of what they see.

 Visual perception is the brain's ability to interpret and make sense of what it sees, giving it meaning. It breaks down into several parts: 

  • Visual discrimination, spotting similarities and differences between objects.

  • Visual memory, recalling something visual after it's gone from sight.

  • Visual-spatial relationships, understanding where objects sit in space. 

  • Form constancy, recognising a shape regardless of its size, angle, or setting.

  • Figure-ground discrimination, picking an object out from its background.

  • Visual closure, recognising something even when part of it is missing.

  • Visual orientation, knowing whether something is facing left or right.

Signs of ocular and visual problems: a checklist for parents.

These are common signs worth watching for and spotting them early can help before your child becomes averse to reading. While some of these signs exist in isolation, noticing 5 or more may indicate that there are visual processing problems: 

  • Regular headaches, especially above or around the eyes 

  • Blurred or double vision 

  • Getting tired easily when reading or writing 

  • Tilting or moving the head while reading or writing 

  • Covering, squeezing, or squinting one or both eyes 

  • Losing their place, skipping or re-reading the same words 

  • Complaining of tired or watery eyes, or rubbing them often 

  • Getting easily distracted, with poor concentration from the effort of focusing 

  • Eye strain, including from screens 

  • Poor coordination and spatial awareness up close (though often fine at sports) 

  • Sensitivity to light 

  • Needing a finger or marker to keep their place 

  • Saying that words seem to ‘move’ or ‘jump about’

  • Reading very slowly and with visible effort 

  • Untidy, irregular handwriting 

  • Trouble copying from the board 

  • Poor sense of direction or orientation 

  • Motion sickness 

How to improve visual skills for reading at home and school.

Simple activities like throwing and catching a ball are among the most effective ways to help. If a child can't throw yet, rolling the ball works just as well as a starting point. These games build ocular motor and binocular skills in a natural, playful way, and because these foundations respond quickly to practice, progress and transfer into academics can be surprisingly fast.

When to seek therapy for reading difficulties.

Visual difficulties often go hand in hand with balance problems. Poor balance can disrupt a child's sense of left and right, which affects spatial awareness, including the ability to tell letters like ‘b’ and ‘d’ apart. If a child struggles to tell such letters apart, learning phonics becomes much harder. Parents sometimes assume this points to dyslexia, when it may in fact be a balance-related visual issue, either alongside dyslexia or entirely on its own. 

A diagnosis isn't required for support; different children need different approaches. For an autistic child, or one in a specialist unit, we'd usually look at visual difficulties as part of reflex integration therapy, particularly where language development is also delayed. This builds ocular motor skills and joint attention, which in turn supports both receptive and expressive language. 

For children in mainstream school with a suspected neurodivergent profile: dyslexia, ADHD, or autism, we'd recommend addressing this through one-to-one therapy that covers all areas together: vision, processing speed, and balance. This means a tailored programme that connects visual skills directly to their phonics and reading progress.
